Lake Superior Life Care & Hospice is seeking a dedicated and compassionate Social Worker to support patients and families navigating some of life’s most meaningful moments. Position Summary

The Hospice Social Worker plays a vital role on the interdisciplinary team, providing emotional support, resource coordination, and counseling to patients and families facing serious, life-limiting illness.

This position focuses on helping individuals navigate complex social, emotional, and practical challenges to enhance quality of life during hospice care.

What You’ll Do

  • Provide compassionate social work support to patients and their families
  • Conduct comprehensive patient assessments to identify psychosocial needs, preferences, and support systems.
  • Provide individual and group counseling sessions utilizing behavioral health techniques such as cognitive behavioral therapy and motivational interviewing.
  • Assist patients and families with emotional support, crisis intervention, and coping strategies during end-of-life care.
  • Coordinate with healthcare providers to develop personalized care plans that address social determinants of health and facilitate access to community resources.
  • Maintain accurate medical documentation in compliance with HIPAA regulations, including detailed case notes and progress reports.
  • Facilitate communication between patients, families, and medical teams to ensure holistic understanding of care goals.
  • Participate in interdisciplinary team meetings to contribute insights on psychosocial aspects of patient care.
  • Assist with advance care planning, including advance directives, DNR, and related forms
  • Help patients and families access community resources and needed services
  • Support families in navigating insurance, Medicare, and Medicaid processes
  • Provide education on coping, grief, and adjustment to illness
  • Collaborate closely with the interdisciplinary hospice team
  • Participate in care planning meetings and ongoing team communication
  • Travel to patient care settings including homes and facilities throughout Marquette County

Required:

  • Licensed Social Worker (LMSW or LCSW preferred; comparable licensure considered)
  • Valid driver’s license, reliable vehicle, and insurance

Preferred:

  • 1–2 years of healthcare, hospice, or related experience

Schedule & Work Environment

  • Monday–Friday, day shift
  • No nights or weekends
  • In-person role with travel within the service area

Current Cardio Pulmonary Resuscitation (CPR) certification required

Current Michigan Driver's License

Previous experience in hospice and/or home health are preferred, but not required.
