Life Care Coordinator

(Part time)

Join our team of passionate, caring professionals!

The Life Care Coordinator is a part-time position. This person is responsible for the coordination and administration of our Life Care services. This position oversees the Adult Day Services program and In-home Respite program and will work with the entire Lake Superior Life Care & Hospice Team to create new Life Care programs to serve our community.

Duties and Responsibilities

  • Develops relationships with potential families, caregivers, and participants, sharing the Lake Superior Life Care & Hospice services and story. Serves as an information and referral coordinator for Life Care Services and enrolls or connects patients and families to the appropriate staff member for enrollment. Performs info-shares, admissions and assessments into Life Care Programs, and coordinates care for clients per services needed.
  • Completes service plans and assessments in collaboration with the Adult Day Services Coordinator.
  • Develops, reviews and updates all life care policies and procedures in conjunction with the CEO.
  • Ensures contractual compliance with contractors, policies and any federal or state laws. Communicates all pertinent information to the CEO and other members of the team as appropriate.
  • Serves as the Life Care Outreach Coordinator to develop and maintain a partnership for full community participation to include: social service/community organizations, education/development centers, faith based organizations, businesses, government, media, health organizations, etc.
  • Maintains accurate records and completes reporting as required for compliance with partner organizations and granting agencies.

Experience

  • Licensed Professional; LPN/Social Work. Bachelor’s degree preferred.
  • Experience in coordination of team projects and providing direction and support to staff, participants and families.
  • Proficiency with technology, including electronic medical record systems and Microsoft Office Suite.
