Position: RN Case Manager / Visit Nurse

Schedule:

• .8 FTE
• 8:00am–4:30pm
• Weekend On-Call Rotation (may vary by need)
• Flexible scheduling + competitive on-call pay

What You’ll Do:

• Visit patients in their home setting—wherever they call home
• Deliver compassionate, patient-centered hospice care
• Manage symptoms, educate families, and provide holistic support
• Collaborate with a deeply committed team that values your voice and autonomy

Current Michigan RN License required at time of hire.
