RN Case Manager/Field Nurse - HIRING BONUS!

As a Hospice RN Case Manager, your ability to manage client care with specific knowledge and experience in bedside care, symptom management, crisis intervention and family intervention are feature assets for this high-profile nursing position. In addition to excellent written/verbal communication, problem solving and decision making abilities, our candidate has a demonstrated experience developing and maintaining rapport with patients and families, and the ability to work well with an interdisciplinary team. This position utilizes the Case Management process to facilitate comprehensive nursing assessment, planning and care to maximize the comfort and health of patients and families in accordance with the interdisciplinary plan of care and regulatory requirements.

Current openings for .8 and Part Time/Contingent positions. 

  • Weekend call every 3rd weekend
  • Extensive benefits package including medical, dental, and vision
  • Current Cardio Pulmonary Resuscitation (CPR) certification required
  • Previous experience in hospice and/or home health are preferred, but not required.
  • Michigan RN License required, CHPN certification a plus.

Human Resources Coordinator

The Human Resources Coordinator is instrumental to Lake Superior Life Care & Hospice performing administrative duties that are central to operations, productivity, sustainability and employee satisfaction. This position plays a pivotal role in maintaining the non-profit’s human resources framework as they report to the Chief Executive Officer.

  • 20 Hours Per Week
  • Flexible Schedule

Human Resources Coordinator Job Responsibilities:

  • Enhances the organization’s human resources by planning, implementing, and evaluating employee relations and human resources policies, programs, and practices.
  • Maintains the work structure by updating job requirements and job descriptions for all positions.
  • Supports the organization’s staff by establishing and performing a recruiting, testing, and interviewing program on candidate selection; conducting and analyzing candidates, performing background checks, scheduling pre-employment physicals and exit interviews; and recommending changes.
  • Establishes and manages a comprehensive orientation program to educate employees regarding onboarding and policies and procedures.
  • Develops and manages a pay plan by studying periodic pay surveys; prepares pay budgets; monitors and recommends pay structure revisions.
  • Learns and processes payroll in conjunction with the finance department and maintains payroll records.
  • Ensures planning, monitoring, and evaluation of employees work results are completed in a timely manner.
  • Provides training to managers to coach and discipline employees; scheduling management conferences with employees; handles employee grievances; and coaches employees and supervisors. Arranges for training and development programs for employees.
  • Answers internal and external queries that are relevant to the HR department including workers compensation, FMLA, unemployment, garnishments etc.
  • Implements employee benefits programs and informs employees of benefits by studying and assessing benefit needs and trends; recommending benefit programs to management; directing the processing of benefit claims; obtaining and evaluating benefit contract bids; awarding benefit contracts; and designing and conducting educational programs on benefit programs.
  • Ensures legal compliance by monitoring and implementing applicable human resource federal and state requirements, conducting investigations, maintaining records, and representing the organization at hearings.
  • Enforces management guidelines by preparing, updating, and recommending human resource policies and procedures.
  • Maintains and retains current and historical human resource records by designing/maintaining a current filing system and retrieval system for previous records.
  • Cultivates professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal networks, and participating in professional societies.
  • Other various duties related to Human Resources as assigned.

Education, Experience, and Licensing Requirements:

  • Bachelor’s degree from a four-year college or university or similar work experience required
  • 5+ years of experience in progressively responsible human resource roles
  • 3+ years of experience in a supervisory role
  • PHR/SPHR certification preferred

Hospice Home Care Aide - HIRING BONUS!

Part Time

The Hospice Home Care Aide provides personal care services to terminally ill patients under the direction the RN Clinical Supervisor and Clinical Director.

Duties include but are not limited to:

bathing, dressing, shaving, exercising, meal preparation, feeding and light house keeping

The ideal candidate will have a high level of comfort with hands on care. Other qualities include: an ability to establish effective working relationships, utilize good judgement in dealing with patients and staff, and exhibit initiative and resourcefulness when dealing with others.

CNA certificate is required.
