The Hospice Home Care Aide provides personal care services to terminally ill patients under the direction the RN Clinical Supervisor and Clinical Director.
Duties include but are not limited to:
bathing, dressing, shaving, exercising, meal preparation, feeding and light house keeping
The ideal candidate will have a high level of comfort with hands on care. Other qualities include: an ability to establish effective working relationships, utilize good judgement in dealing with patients and staff, and exhibit initiative and resourcefulness when dealing with others.
CNA certificate is required.
We provide comprehensive training and ongoing mentorship for all of our staff and especially for those who may be new to hospice.

Responsibilities
As a Spiritual Care Counselor you will be responsible for providing spiritual care services to patients and families/caregivers with diverse religious, cultural and spiritual preferences, either directly or through coordination of care with other spiritual counselors, clergy and hospice team members.
You will collaborate with a team of providers that may include a hospice RN, social worker, aide, chaplain, and volunteers to provide world-class care for your patient
Assuring spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner
Facilitating the development of the individualized plan of care by participating in the comprehensive assessment to meet identified spiritual needs
Assists members of the interdisciplinary team in understanding the significant spiritual concerns related to the patient/family response to dying
Serves as liaison and resource for community spiritual leaders in dealing with hospice patients/families
Being available to staff for bereavement support
Participate in Hospice performance improvement programs
Maintain high level of customer satisfaction with our clients, families and caregivers
Consistently and accurately update Electronic Medical Records
Planning periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team
Carrying out the mission and values of Lake Superior Life Care & Hospice

As a Hospice RN Case Manager, your ability to manage client care with specific knowledge and experience in bedside care, symptom management, crisis intervention and family intervention are feature assets for this high-profile nursing position. In addition to excellent written/verbal communication, problem solving and decision making abilities, our candidate has a demonstrated experience developing and maintaining rapport with patients and families, and the ability to work well with an interdisciplinary team. This position utilizes the Case Management process to facilitate comprehensive nursing assessment, planning and care to maximize the comfort and health of patients and families in accordance with the interdisciplinary plan of care and regulatory requirements.
