Care that is Goal-Centered

We listen to the patient, the caregiver, and the family. The patient directs his or her own personal care plan. We find out what their goals are and build a care plan around that to make sure they are getting the care they want and need. The patient’s goals form the focus of the care team who serve the patient and their family. Hospice services include a whole array of choices; not only medical services, medications, and equipment, but also emotional and spiritual support for the patient and the family.

The Hospice Team

Each patient and their families have a team of skilled professionals to help during this trying time. They include physicians, nurses, social workers, spiritual counselors, hospice aides, volunteer coordinator/volunteers, and bereavement coordinator. Who pays for Hospice Care?

Medicare, Medicaid and most private insurances cover hospice services. With this coverage there are minimal out-of-pocket expenses, even for medications, equipment and other high-cost items that are often needed in the last months of life.

The support and care giving that our staff and volunteers provide and the financial relief that the Medicare/Medicaid hospice benefit offers reduces your family’s burden. In most cases, there are virtually no bills, no claims and no financial surprises. Medicare and Medicaid will continue to provide their normal benefits for care unrelated to a patient’s primary diagnosis.

A person will never be denied hospice care from LSH just because they or their insurance company are unable to afford hospice care. We are a non-profit organization who takes any surplus of revenue at the end of the year and puts it back into patient care.
